What is Hospice?
Hospice is a place of rest for the weak and weary. Hospice is a service to help individuals enjoy creating the last memories of their life. The goal of hospice is to help the patient live a pain free alert life, in their final days.
What does Hospice Provide?

- Chaplain
- Medications related to the hospice diagnosis
- Medical equipment related to the hospice diagnosis
- Round the clock support for the patient and their family members
- On call nurses
- Home Care Connection also provides Hospice Message Therapy
How is Hospice Paid For?
The Medicare hospice benefit covers 100% of the hospice costs. The hospice benefit provides skilled nursing, certified nursing assistants, a social worker, medical equipment, supplies, and any medications related to the disease process.
Mending Hearts Grief Recovery Group
Healing a broken heart is the hardest thing a person can do. There is not a shortcut and no way to heal without grieving. A person's grief is very personal and everyone's journey is different. The Home Care Connection & Hospice staff includes grief counselors to help you along the way. Mending Hearts Grief Recovery also offers memorials for families where they may meet others who have been through hospice and send a balloon with a special message off or a even plant a flower for their loved one.
Massage Therapy
The Home Care Connection & Hospice offers massage therapy for
hospice patients and their primary care giver. A licensed massage
therapist can be scheduled through our office to visit your home, at no
cost to you. Massage therapy helps the patient relax, increases
circulation, improves mobility, and decreases pain.
