“Our patients and their families start out as friends and they become family forever.”

What is Hospice • Grief Support • Hospice Volunteers
What is Hospice?
Hospice is a service available when medical care cannot offer a cure; hospice provides care, comfort, and support for persons with life-limiting conditions as well as their families. The hospice team works to make the person comfortable and relieves their symptoms and pain for the entire length of their illness.
The Hospice Team
Hospice care is a family-centered team approach that includes, a doctor, nurse, social worker, councelor, chaplain, certified massage therapists, home-health aide and trained volunteers. We work together, focusing on the physical, psycological, social, and spiritual needs the patient.
Our Hospice team will provide the following services to individuals in the home. These may include:
- Management of Pain and Other Symptoms
- Emotional and Spiritual Supportive Care
- Provide Medications, Medical Supplies, and Equipment
- Inpatient Care When Symptoms Become Too Difficult to Manage At Home
- Bereavement Support and Counseling to Family Members for 18 Months
- Teach Family Members Skills to Help Provide Care
Pain Control
Pain control is helping the patient to stay as symptom-free as possible and allow the patient to be in control of their life. Home Care Connection and Hospice also has two on-staff massage therapists to help control pain for the patient.
In-Home Hospice
Home Hospice keeps the patient in their home and with their family. The On Call Nurse is available to the patient at all times, and if the patient is in need of anything, the on call nurse is just a phone call away.
Grief Support
Grief Support is a coming together of individuals or a one-on-one with our Chaplain to talk about the grief process. Your illness, or loss of a loved one, can cause you to be sad, depressed, angry, hurt, or push your pain deep inside. We want to be here for you. Through our Hospice and Home Care, our chaplain is here for those who are hurting. He wants to walk alongside you as you travel down this path called grief. There is not a “right” way to grieve, and no two people grieve the same, but the one thing everyone has in common in their grief process is that they need understanding and support. If you are grieving and need help, please call. Andy is available to everyone in the community. Let us help you on your journey of grief.
Spiritual Care
The Home Care Connection and Hospice’s chaplain is available to all of our patients to help them complete their journey in life. If the patient has any spiritual questions, he is available to answer them and work with the patient to come to a peace in their spiritual life.
Family Care
The Home Care Connection and Hospice stays with the patient’s family after they have entered into rest. The chaplain offers a Celebrate Recovery group for any individual who is recovering from grief as well as classes to teach them how to deal with their grief. The Home Care Connection and Hospice has two on-staff massage therapists to help relieve stress and tension once a month for the caregiver.
Hospice Volunteer
A Hospice Volunteer offers comfort and support for Hospice patients.
A Hospice Volunteer cares for the patients and may give personal time to the caregivers.
The Hospice Volunteers may also cook meals, bake goodies, clean house, or many other forms of care.
The Hospice Volunteers are screened and trained for many hours prior to going into the patients home.
